The testing in the test environment should not be limited to the changed product, but should also include closely related products. If related products appear to be affected this may mean that these will need to be updated or changed as well, meaning that this change cannot be completed until change requests for those systems are approved. In most cases this should have been detected in the planning phase, but inevitably some cases slip through the cracks.
